In order to make it work at your site, you've got to put both nfmca.jar and NFMCA.html(or the file, into which you inserted it) into the same directory and set the port attribute of corresponding tags(see NFMCA.html's source) to your MUD server's port. This assumes that your web-server has the same IP address as your MUD server. If this is not true, you'll have to use proxy or something else.
